Start Living Your Life Again with PACE@Home

The number of people worldwide living with dementia is growing at an alarming rate. In 2020, there were 55 million cases, but that number is expected to nearly double by 2030, reaching 78 million. It's a difficult reality that dementia patients often live with the disease for many years. Family members and other caregivers often face challenges in providing the best care to their loved ones, partly because the progression of this disease can be unpredictable.

Many are unaware about how PACE@Home can provide extra support and help their loved one continue living at home, while still receiving medical care and other assistance from trained professionals.

What is PACE?

PACE helps seniors live independently by providing them with a team of professionals who are focused on their needs. The program offers medical and support services through an integrated approach to care that helps make it possible for those who are aging to continue to live comfortably in their community rather than a skilled nursing facility.

PACE Eligibility

To become a PACE@Home participant, residents must meet the following requirements:

  • Age 55 or older

  • Determined by the state of North Carolina to be in need of nursing home care

  • Have the ability to live safely in the community at the time of enrollment with PACE@Home service

  • Reside in the PACE@Home service area within the ZIP codes listed below:
    o   Alexander County (28601, 28630, 28636, 28678, 28681)
    o   Burke County (28601, 28602, 28612, 28619, 28628, 28637, 28655, 28666, 28671, 28680, 28690)
    o   Caldwell County (28601, 28630, 28633, 28638, 28645, 29667)
    o   Catawba County (28037, 28168, 28601, 28602, 28609, 28610, 28612, 28613, 28650, 28658, 28673, 28682)
    o   Lincoln County (28037, 28092, 28168, 28650, 28673)

PACE Benefits

The PACE program offers a number of benefits. Doctors, nurses, physical therapists, social workers and dietitians work with participants to create an individualized plan that can include everything from social services to transportation to medical care. Here’s a complete list of services:

How Much Does PACE Cost?

As a federally funded program, PACE@Home provides comprehensive medical care and is administered by the state through Medicaid waivers. It can be a cost-effective alternative for those who need a wide variety of medical care, along with food and transportation services. PACE@HOME is a program of All-Inclusive Care for the Elderly that allows individuals in need of skilled nursing home care to remain in their home or community setting. PACE@Home is a voluntary, medical/social-based model of care for those who are eligible that creates an individualized plan of care for each participant. This person-centered plan of care is developed, reviewed and updated regularly by an interdisciplinary team to ensure that each participant’s needs are addressed.
