The PACE@Home program has been providing all the necessary medical and supportive services to meet your needs for more than 10 years. These may include:

Adult Day Health Services

We are a leading provider of adult daycare, offering an innovative solution to families who are looking for someone to help care for their loved one. Our specially trained staff ensures that your loved one receives healthy meals and personal attention while attending to their functional needs, such as bathing and dressing.

In Home Services

We offer in-home visits for enrolled participants to help them stay healthy in the safety and convenience of their home.

Medical Care and Medical Specialists

Offering an integrated approach to medical care, the PACE@Home team is on call 24/7 and has access to specialists who care for patients experiencing a variety of medical conditions.

The PACE@Home program will provide each participant’s sole service provider and guarantees access to services, but not from a specific provider. A participant may be disenrolled in the PACE@Home Program voluntarily or involuntarily due to cause or if they become deceased.

Dental, Vision, Hearing and Foot Care

We offer a one-stop shop for participants, from dental care and vision screenings to hearing tests, foot exams and checkups.

Meal Planning and Nutrition Services

We offer a number of services to help participants with their dietary needs, including meal planning and nutrition counseling, which can be especially helpful for those who have been diagnosed with chronic diseases or obesity-related issues such as diabetes or high cholesterol.

Home Health Care and Personal Care

We offer healthcare at home and assistance with daily activities, including dressing and bathing. Healthcare providers also help with minor injuries and therapies to keep participants moving while improving their strength and balance.

Medications

We can help participants manage and organize their medications. We also remind them when it's time for another dose, and stay on top of missed doses, refill reminders and upcoming doctors’ appointments.

Social Services

Whether it be an urgent need like applying for food stamps or other government assistance programs, or a less urgent need such as caregiver support, we can help participants and their families get the resources they need.

Respite Care

We offer the necessary support to ensure that loved ones are well cared for, so family caregivers can take a break and focus on their own health and wellbeing. You can find peace of mind knowing someone else is there for your loved one when you aren't available or able.

Physical, Occupational and Speech Therapy

One of the benefits to PACE@ Home's physical, occupational and speech therapy is that it offers personalized treatments for each individual participant. The entire team works together with your loved one on a treatment plan based on their goals, which are designed to be achievable, yet challenging enough to keep them engaged.

Recreational and Social Activities

We offer a range of activities, from low-intensity sports to social gatherings, to help your loved one stay as active and healthy as possible.

Transportation

A variety of transportation services are available, including rides to medical appointments and the PACE@Home center.

Hospital, Emergency Services and Nursing Facility Care

PACE@Home is a great option for families to make sure your loved ones are well cared for. We provide a variety of services to assist participants in receiving the care they need, whether that’s in a hospital, ER, immediate care center or long-term care facility.
